The website feature real-life accounts of issues such as effect on work and education, social life and relationships, consulting health professionals and treatment.

Pre-Registration with the practice

If you wish to pre-register click on the links below to open the two form you need to register with us. When you have completed all of the details click on the "Send" button to mail your form to us. When you visit the surgery for the first time you will be asked to sign the forms to confirm that the details are correct. Please fill in both the pre-registration form and the online medical questionnaire.

To help us locate your existing medical records it is really important that you complete your NHS number on this form. You can obtain this from your previous GP.  It is not your National Insurance number.

The practice does not discriminate on the grounds of race, gender, social class, age, religion, sexual orientation, appearance.

You may print off a registration form, fill it out and bring it in with you on your first visit to the practice

New Patient Medical Questionnaire

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records.
